DOH-Pasco Asks Pasco County Residents to Take the Community Health Needs Assessment Survey

Pasco County, Fla.- The Florida Department of Health in Pasco County (DOH-Pasco) wants to hear from you! The Community Health Needs Assessment (CHNA) Survey has been released to the public and DOH-Pasco is asking all community members to complete the survey.

The 2024 Community Health Needs Assessment (CHNA) collects data from Pasco County residents on nutrition, physical activity, access to care, and mental health. The survey is available in English and Spanish at bit.ly/49jcn1t or by scanning the QR code below. The survey is open through April 15, 2024.

Data collected from the CHNA survey will help DOH-Pasco better understand the needs of the community and will be used to develop a community health improvement plan, addressing the issues residents are most concerned about.

“Your voice helps us prioritize health needs in our community. Knowing what is urgent encourages awareness, action, and improvement,” said DOH-Pasco Health Officer Trang Chitakone.

To participate in the survey, you need to be 18 years old or older and live in Pasco County. The survey comprises of 16 main questions, with additional follow-up questions based on your responses. Additionally, there are eight demographic questions included. Completing the survey should take approximately 10 minutes.
