The Community Health Assessment Resource Tool Set (CHARTS) helps Florida's counties and communities locate data they need for community health assessments and plans. 

CHARTS is developed and maintained by the Florida Department of Health, Division of Public Health Statistics and Performance Management, Bureau of Community Health Assessment.
