Birth Certificates

Florida Department of Health in Pasco County

Florida law restricts the issuance of birth certificates to those eligible applicants who provide VALID photo identification. Acceptable forms of identification are Driver's License, State ID Card, Passport or Military ID Card. Expired forms of identification will not be accepted. If a person does not have a valid photo identification, DOH-Pasco Vital Statistics will accept a combination of three different forms of identification, please call for specific examples. 

Florida birth certificates are not public record. They only become public record 125 years following the birth.

Who can request a birth certificate?

Births less than 125 years old are confidential and may only be issued to:

  • Registrant (name on the record) if of legal age (18)
  • Parent(s) listed on the Birth Record
  • Legal guardian (must provide guardianship papers)
  • Legal representative of one of the above persons (must provide documentation)
  • By Court Order (must provide copy of court order)
  • Upon receipt of registrant’s death record, a birth certificate marked “Deceased” may be issued to the spouse, child, grandchild, or sibling, if of legal age. Please notate “Registrant Deceased” on the application and include a photocopy of the death certificate.

In the case of a deceased registrant, upon receipt of the death certificate of the decedent, the birth certification can be issued to the spouse, child, grandchild, sibling, if of legal age, or a legal representative of any of these persons, as well as to the parent.

Florida death records from 2009 - present are available with some restrictions for $10 per copy.

Vital Statistics staff also provides assistance in explaining how to obtain out-of-state records, or filing record amendments (corrections, paternity acknowledgements, commemorative birth certificates, apostille forms and putative father application).

  • Commemorative birth- Birth Certificate for a person seeking a decedents birth certificate for unofficial purposes
  • Putative Father - application to flag a child's birth record to acknowledge a possible claim for custody of the child for legal purposes

Please Note: The birth certificate for a newborn is not automatically mailed to the parent after birth. The parent may apply for the birth certificate through a Vital Statistics office and pay the proper fee.

If the registrant was not born in Florida, visit the National Center for Health Statistics website.

Payment Methods:

  1. The fee for one certified copy of a Florida birth record is $12.00 and $8.00 for each addition copy of the same record when ordered at the same time.
  2. Optional Purchase: Protective Plastic Covers for Birth and Death Certificates: $3.00 each.
  3. Walk-in Requests, fees are payable by cash, money order, cashier's check, debit card, credit card, Tap to Pay and Apple Pay.
  4. Mail-in Requests, fees are payable by money order, cashier's check, Visa or MasterCard. (Please do not send cash by mail). Money orders/cashier's checks should be made payable to DOH-Pasco. Shipping cost is $10.00.

Completed requests will be processed on date of receipt and shipped via FEDEX priority overnight with a tracking number. Orders will be returned unprocessed if the requests are incomplete.
