The Community Health Improvement Plan (CHIP) was devised by analyzing, sharing, and reviewing local and state-level data that reflect the health of the community. Recent community meetings, collective impact meetings, have identified focus areas to address collectively.
To improve health, Pasco’s Community Health Improvement Plan will focus on the following areas: 1) Health and Well‐Being; 2) Healthy Babies; 3) Infectious Disease; and 4) Access to Care over the upcoming years.
